1. For a list of companies that can match your contributions to GAOCT, contact your employer's Human Resources office to determine if your company matches employee donations. |

2. Complete the donor section of the matching gift form, and if the company does not directly send it to GAOCT, please submit it, along with your check or credit card authorization, to:
